Chronic Pain in Clients with Substance Abuse & the Role of Opioids

Chronic Pain occurs as a complicated web of emotions and physical symptoms. The most common way to treat pain is to use opioid medications, which actually complicate the course of chronic pain. For some individuals who develop dependence/ addiction to these medications, management and treatment for their condition can become much more complicated. This session will review the interactions of pain, suffering and addiction as they interface in your clients with suggestions for intervention and treatment.

Learning Objectives:

Describe principles of chronic pain, and the principles of modern pain neuroscience.
Discuss an overview of the opioid epidemic and its impact on clients with co-occurring chronic pain.
Discuss techniques to manage opioid dependence and the experience of recovery from chronic pain with clinical methods including mindfulness, CBT, DBT, ACT and Pain Reprocessing Therapy.

Colin Ross, MD

Addictions as the Avoidance of Trauma

In this talk, Dr. Ross will explain the treatment of addictions within his Trauma Model Therapy (TMT); a broad range of addictive behaviors are included within the model. The core principles of TMT include: the problem of attachment to the perpetrator; the locus of control shift; the problem is not the problem; just say ‘no’ to drugs; addiction is the opposite of desensitization; and the victim-rescuer-perpetrator triangle. All addictions are seen as avoidance strategies within TMT, as ways to avoid bad feelings on the inside and bad situations on the outside. The treatment is not focused primarily on the addiction itself but rather on what is being avoided and developing healthy self-regulation and coping strategies so that the need for the addiction diminishes. Much of what is being avoided is the conflicts, cognitive errors and symptoms arising from unresolved trauma – diagnostically, this includes depression, complex PTSD, and dissociative disorders, as well as a wide range of unhealthy coping strategies. Treatment strategies and techniques will be illustrated with case examples.

Mel Pohl, MD

Dr. Pohl is certified by the American Board of Addiction Medicine (ABAM) Dr. Pohl is a Clinical Assistant Professor in the Department of Psychiatry and Behavioral Sciences at the University of Nevada School of Medicine.

He is the author of A Day without Pain, revised edition (Central Recovery Press, 2011) and The Pain Antidote – Stop Suffering from Chronic Pain, Avoid Addiction to Painkillers, and Reclaim Your Life (DaCapo, 2015). Dr. Pohl filmed a show for PBS on chronic pain which aired around the country in 2016.

Colin A. Ross, M.D.

Colin A. Ross, M.D. completed medical school at the University of Alberta and his psychiatry training at the University of Manitoba in Canada. He is a Past President of the International Society for the Study of Trauma and Dissociation, and is the author of over 225 papers and 32 books. He has spoken widely throughout North America and Europe, and in China, Malaysia, Australia and New Zealand. He has been a keynote speaker at many different conferences, and has reviewed for over 30 different professional journals.

Dr. Ross is the Director of hospital-based Trauma Programs in Denton, Texas, Torrance, California and Grand Rapids, Michigan. He provides cognitive therapy groups at all three locations, in person in Texas and by video-conference in Michigan and California. He has been running a hospital Trauma Program in the Dallas area since moving to Texas in 1991.

Dr. Ross’ books cover a wide range of topics. His clinical books focus on trauma and dissociation and include: Dissociative Identity Disorder. Diagnosis, Clinical Features and Treatment of Multiple Personality, Second Edition (1997); Schizophrenia: Innovations in Diagnosis and Treatment (2004); The Trauma Model: A Solution to the Problem of Comorbidity in Psychiatry (2007); Trauma Model Therapy: A Treatment Approach for Trauma Dissociation and Complex Comorbidity (2009); Structural Dissociation: A Proposed Modification of the Theory (2013); and Treatment of Dissociative Identity Disorder: Techniques and Strategies for Stabilization (2018).

Dr. Ross has published a series of treatment outcome studies in peer-reviewed journals, which provide evidence for the effectiveness of Trauma Model Therapy. Many of his papers involve large series of cases, with original research data and statistical analyses, including a paper entitled ‘Trauma and Dissociation in China” in the American Journal of Psychiatry.

Besides his clinical psychiatry interests, Dr. Ross has published papers and books on cancer and human energy fields, as well as literary works including essays, fiction, poetry and screenplays. He has several different hobbies including travel.
