Reconsidering Trauma: Treatment Advances, Relational Issues & Mindfulness

Modern approaches to trauma tend to focus on the psychopathology of adverse experience. However, trauma exposure is not the only contributor to posttraumatic suffering. Trauma will be described as a normal part of human experience, with chronic posttraumatic suffering more likely when trauma-related memories, thoughts, and feelings are experienced as unacceptable and overwhelming, and then excluded from awareness through thought suppression, behavioral avoidance, distraction, or numbing. Yet, research indicates that avoiding pain actually increases symptoms and distress, and directly experiencing and allowing pain ultimately reduces its power. This Pain Paradox, long appreciated in Buddhist psychology, suggests a reworking of traditional therapeutic models so that awareness can increase in the context of carefully titrated engagement with — and less encumbered consideration of – painful history. John Briere will outline the clinical implications of this perspective, emphasizing the role mindfulness, metacognitive awareness, and a compassion-focused therapeutic relationship.

John Briere, Ph.D.

John Briere, Ph.D., is Professor Emeritus of Psychiatry at the University of Southern California Keck School of Medicine. He has spent decades treating and researching the effects of extended childhood maltreatment, sexual and physical assaults, torture, and severe burns. A past president of the International Society for Traumatic Stress Studies, Dr., Briere is recipient of the Award for Outstanding Contributions to the Science of Trauma Psychology from the American Psychological Association and the William N. Friedrich Lecturer: Outstanding Contribution to the Field of Child Psychology from the Mayo Clinic. He is author or co-author of over 140 articles and chapters, 15 books, and 9 trauma-related psychological tests, and has been Remote Faculty at the Institute for Meditation and Psychotherapy since 2013. His 2019 book with Guilford Press is Treating risky and compulsive behavior in trauma survivors. His website address is johnbriere.com.
