Addressing Sexual Behavior Problems in Trauma Survivors

Risky sexual behavior is an unfortunately common problem for childhood sexual abuse survivors and/or those with early experiences of neglect or disattunement. In this workshop, the trauma/attachment basis for compulsive sexual behavior will be outlined, and a nonstigmatizing, compassionate treatment approach presented. Among the topics in this section are processing shame issues, dealing with sexualized responses to the therapist, and harm reduction interventions to reduce risk of sexually transmitted infections (including HIV/AIDS) and the possibility of sexual revictimization arising from indiscriminate partner choice.

Learning objectives:

1. Discuss the most typical counter transferential reactions to client risky behavior.
2. Describe one way that the therapist can deescalate client sexualized behavior in the session.
3. Outline three central aspects of trigger management for sexually reactive behavior.

Reactive Avoidance: A New Perspective on Behavioral Addictions

Survivors of trauma and early attachment disturbance are at elevated risk of a variety compulsive behaviors, ranging from gambling to binge eating. These difficulties are often seen as symptoms of a behavioral addiction or impulse control disorder. However, emerging research indicates that they are more accurately understood as dysfunctional coping responses to triggered childhood memories and inadequate emotional regulation capacities. Calling on his newest book, John Briere presents an integrated approach to repetitive, dysfunctional behavior, the Reactive Avoidance model (RAm). This approach directly targets the distress reduction dynamic, with interventions such as trigger management, metacognitive awareness, emotion surfing, and harm reduction.

Learning objectives:

1. Outline two central principles of the Reactive Avoidance model.
2. Discuss the etiology of Distress Reduction Behaviors from a coping perspective.
3. List the major steps of trigger management.

John Briere, Ph.D.

John Briere, Ph.D., is Professor Emeritus of Psychiatry at the University of Southern California Keck School of Medicine. He has spent decades treating and researching the effects of extended childhood maltreatment, sexual and physical assaults, torture, and severe burns. A past president of the International Society for Traumatic Stress Studies, Dr., Briere is recipient of the Award for Outstanding Contributions to the Science of Trauma Psychology from the American Psychological Association and the William N. Friedrich Lecturer: Outstanding Contribution to the Field of Child Psychology from the Mayo Clinic. He is author or co-author of over 140 articles and chapters, 15 books, and 9 trauma-related psychological tests, and has been Remote Faculty at the Institute for Meditation and Psychotherapy since 2013. His 2019 book with Guilford Press is Treating risky and compulsive behavior in trauma survivors. His website address is johnbriere.com.
